MND 評估記錄對應的 Support at Home 與 AT-HM 優先路徑
官方說明，若 MND 已記錄在老年照護評估中，當事人可優先獲得 ongoing Support at Home funding，並在 AT-HM 計劃下獲得 immediate priority；兩項安排都適用於已獲相應資金批准或今後獲批的人。服務商只應記錄與轉介，不應判斷診斷、資格或優先級。
