MND 评估记录对应的 Support at Home 与 AT-HM 优先路径
官方说明，若 MND 已记录在老年照护评估中，当事人可优先获得 ongoing Support at Home funding，并在 AT-HM 方案下获得 immediate priority；两项安排都适用于已获相应资金批准或今后获批的人。服务商只应记录与转介，不应判断诊断、资格或优先级。
