Support log basics
A support log records the support delivered to an individual participant. It should be consistent with the roster, invoice or claim information, and any related case note or progress note.
- Participant name or identifier and date of support.
- Amount, quantity, or hours where relevant.
- Support type and activity details.
- Participant, nominee, child representative, or legal guardian sign-off where required by the provider process.
