Using this resource

This checklist supports preparation; it is not a service agreement or legal approval. Use the official guidance when preparing the actual written agreement.

Confirm any decision-maker's active authority. Allow the participant time and support to understand the agreement; being a registered supporter alone does not establish authority to sign.

For ongoing services, track agreement review at least every 12 months and on participant or active decision-maker request. Keep unresolved questions and the follow-up owner visible.

Who it is for

Care coordinators, intake staff, provider managers, and small home care providers preparing participant service documentation.

When to use it

Use before starting services, when changing service arrangements, when checking whether a service agreement discussion is complete, or when preparing agreement wording for Support at Home pricing and personal care contribution changes.
