How CaresLink handles information submitted through downloads, contact forms, resource requests, and Provider Profile previews.
CaresLink provides free operational documentation templates and resources for Australian aged care, home care, and NDIS support teams.
Information we collect
When you submit a form, we may collect your name, email address, message, the resource you requested, selected download language or version, and basic technical information such as page URL and referral source. We use this information to provide downloads, respond to enquiries, understand which resources are useful, improve templates, and send relevant updates if you choose to receive them.
Do not send participant names, care notes, health information, identity documents, payment details, or other sensitive case information through a public website form.
Provider profile submissions and browser storage
The Provider Profile Generator saves the completed draft in the browser on the device used to create the preview. The saved draft can include the provider or business name, contact person, email, phone or WhatsApp, WeChat ID, website, ABN, service categories and referral services, service areas, languages, program and availability selections, descriptions, target clients, and source channel. It can remain on that device unless the user or browser clears this site's data.
After the submitter confirms the just-in-time consent statement and submits the form, the site also attempts to send those fields, generated English and Chinese profile wording, form context, page URL, referrer, and submission time through FormSubmit, a third-party form processor, to the CaresLink receiving email system. CaresLink uses this external submission for provider-profile review, contact, and follow-up. It is not marketing consent and does not automatically create a public listing.
Clearing the draft from the device does not remove a submission already forwarded through FormSubmit or the receiving email system. Retention and deletion timing in those external systems is not promised by this public page. Use the manual privacy request path below for access, correction, or removal requests, and do not include participant, health, care-note, identity, payment, or other sensitive case data.
How we use and share information
We may use trusted service providers for website hosting, analytics, email delivery, and form processing. We do not sell personal information.
Access, correction, removal, and account deletion
You can request access to, correction of, or removal of contact details submitted to this public website through the contact page. Describe the public page or form involved, but do not include participant or care-record data. This is a manual request path; the public site does not present an automated privacy or deletion control.
If your request concerns a CaresLink Portal or App account, follow the current account-deletion status and verified support path. The public website cannot inspect, export, or delete a private account. Choosing email updates is optional and is not required to receive a download. Each marketing email includes an unsubscribe option, and you can withdraw your consent at any time.
