Australian support workers often need progress notes that are factual, clear, and useful for the next worker, coordinator, or provider. A good note records what support was delivered, how the person participated, relevant observations, and any follow-up needed.
For everyday support, keep the language plain. Record what you saw, heard, did, and reported. Avoid judgemental wording or assumptions about a person's motivation, diagnosis, or behaviour unless your organisation has a specific approved wording process.
A simple structure is: date and time, support delivered, person's participation, observations, risks or changes noticed, actions taken, and follow-up required. This structure works for community access, personal care, transport, meal preparation, and domestic assistance notes.
Example wording: 'Supported Mary with grocery shopping and meal planning. Mary chose meals for the week and compared prices independently with verbal prompts. No hazards or concerns observed during the outing. Receipts uploaded as per provider process.'
Use progress note templates as an operational starting point only. Your provider's policies, funding rules, participant goals, care plan, and privacy requirements should decide the final wording and storage process.
