Common triggers

A change note may be useful after a fall, repeated declined support, hospital admission, carer change, new equipment need, increased fatigue, changed routine, or service concern.

What to include

Record the change, who reported it, immediate risk, services affected, client preferences, informal supports affected, coordinator action, referral or reassessment consideration, and follow-up date.
