Using this resource
For Support at Home, review the care plan with the participant at least every 12 months and sooner when needs, goals, preferences, risks or services change. See the linked official guidance for other review triggers.
Use the Actions section to record agreed changes, the owner and due date. When the care plan changes, review the individualised budget and give the participant or registered supporter an updated copy of the plan.
This template records a review meeting for an existing care plan. Adapt it to your organisation's consent, privacy and escalation procedures; it does not replace the care plan, an assessment or a My Aged Care Support Plan Review. The Restorative Care Pathway uses a goal plan.

Care managers, coordinators, team leaders, and providers completing routine care plan reviews.
When to use it
Use during scheduled reviews, after a change in needs, or when services are being adjusted.
