Support at Home Documentation Templates for Australian Providers

A practical overview of useful documentation templates for providers delivering Support at Home services.

The Support at Home program is now a central part of Australian in-home aged care. Providers need operational documents that help workers and coordinators record service delivery clearly and consistently.

Useful template categories include service agreement checklists, price discussion records, intake forms, home safety checklists, care management contact notes, care plan review templates, and change of circumstances notes.

The Australian Government states that Support at Home replaced the Home Care Packages Program and Short-Term Restorative Care Programme on 1 November 2025. CHSP is scheduled to transition no earlier than 1 July 2027.

Because Support at Home includes program rules and provider responsibilities, templates should be treated as operational drafting aids only. They are not a substitute for the Support at Home manual, legal review, compliance advice, or provider-specific procedures.

For SEO and user experience, each template should have a plain-English title, a short explanation of when to use it, and a download option that does not bury the disclaimer.
