Support at Home Documentation Templates for Australian Providers

The Support at Home program is now a central part of Australian in-home aged care. Providers need operational documents that help workers and coordinators record service delivery clearly and consistently.

The Australian Government states that Support at Home replaced the Home Care Packages Program and Short-Term Restorative Care Programme on 1 November 2025. CHSP is scheduled to transition no earlier than 1 July 2027.
