What to capture

Record the date, time, type of prompt, what the client did after the prompt, any refusal or issue, who was notified, and where the record was stored.

Important boundary

Medication support is sensitive. Workers should only act within their role, training, provider policy, and the person's support plan. The template does not provide clinical or medication advice.
