Medication Prompt Record Template for Australian Support Workers
Operational guidance for medication prompt records in Australian home care and disability support settings.
Medication prompting is a sensitive area. Support workers should only work within their role, training, provider policy, and the person's support plan. A medication prompt record can help document that the agreed prompt occurred.
A simple record may include date, time, medication prompt type, whether the person took action independently after the prompt, any refusal or issue, who was notified, and worker name.
Example wording: '8:00 am medication prompt provided as per support instructions. Client checked Webster pack and took morning dose independently. No issues raised. Record completed and stored according to provider process.'
If a dose is missed, refused, dropped, or there is any uncertainty, workers should follow the organisation's escalation process immediately. A template cannot decide clinical or medication management actions.
Because medication support arrangements vary, providers should review any medication prompt template carefully before use.
