What to include
- Name or role of the person making contact and the date/time.

- Recording unnecessary family conflict or unrelated personal details.
- Promising changes outside your role.
- Leaving the owner and due date unclear.
Example wording
Daughter contacted the service regarding concern about increased fatigue after appointments. Worker advised this would be shared with the coordinator for review. Next step: coordinator to review visit times and call daughter by Friday.
