When to use this scenario

Use this scenario after community access, social participation, shopping, appointments, transport practice, or similar support where the person made choices or practised a skill.

What to include

  • What support was delivered and where it occurred.
  • How the person participated, made choices, used prompts, or practised a goal.
  • Any risk, change, missed task, or follow-up that the next worker or coordinator needs.

What to avoid

  • Judging attitude or motivation without observable facts.
  • Writing a goal link that was not part of the support.
  • Adding unrelated personal details that do not help the record.

Example wording

Supported Mary with grocery shopping and public transport practice. Mary selected items, compared prices, and tapped on independently after one verbal prompt. No hazards or concerns observed during the outing. Follow-up: continue practising the return trip next week.
