Plain-English examples for writing aged care progress notes in Australian home care and Support at Home contexts.

In Australian home care, progress notes help workers and coordinators understand what happened during a visit and whether anything needs follow-up. They should be useful, brief, and connected to the services actually delivered.

Aged care notes should usually record the support provided, the older person's choices and preferences, any changes noticed, and any actions taken under the provider's process. For example, if a person declined assistance, record the offer, the person's decision, and whether anyone was notified.

Example wording: 'Attended scheduled domestic assistance visit. John requested kitchen and bathroom cleaning first. Tasks completed as requested. John reported feeling tired after a morning appointment and chose to rest after the visit. No immediate concerns observed. Coordinator updated about reduced energy level.'

The Support at Home program replaced Home Care Packages and Short-Term Restorative Care on 1 November 2025. Because providers may use different documentation systems and service rules, templates should be reviewed locally before use.
