De-identification assistance needs human judgment
The browser review helps find some direct identifiers and wording risks. It cannot prove that information is fully de-identified.
Remove names, phone numbers, emails, participant numbers, dates of birth and exact residential addresses.
Review indirect clues such as a rare event, exact place or distinctive combination of time and circumstances.
Replace labels such as anxious, aggressive or non-compliant with confirmed observable facts, or leave them out.
Confirmed structured facts are still sensitive operational information and need appropriate local handling.
A drafting aid with firm boundaries
Every result remains Draft - review required. The companion does not decide whether an incident is reportable, assess risk, diagnose a participant, judge goal achievement or evaluate worker or provider quality.

Do I need an account to use the companion?
Yes. Sign in or create an account before entering support facts, running the privacy review, generating or saving a draft.
Does the automated review guarantee de-identification?
No. It helps identify some obvious and indirect risks, but the user must review the cleaned version and decide whether more detail needs to be removed or generalised.
Is the generated wording a completed case note?
No. It is user-reviewed draft wording. Check it against the actual event, your organisation's procedures and the system where the formal record belongs.
Will the companion decide whether something is an incident?
No. It does not determine incident status, reportability, risk, clinical meaning, compliance or service quality.
